Ministerial Resolution No. (21) of 2022 approves the annual report form for medically assisted reproduction centers.

The form includes general information about the center, employee number, medical devices and equipment, including the preventive maintenance of such devices and equipment annually.

The form also includes the types and number of medically assisted reproduction techniques, indicators and rates of pregnancy success, and the age groups of cases that received treatment using medically assisted reproductive technologies within the center.

The form contains a customer satisfaction survey for the services provided by the center. This resolution annulled every other provision that contradicted its provisions.
